Skin tears and similar injuries often require immediate attention, so we prioritize urgent needs by frequently visiting within a few hours to help avoid a trip to urgent care.

Doctor's appointments often involve important discussions that can significantly impact a patient's health goals and interventions, including medication management and personalized care plans. Our nurses can attend these appointments to advocate for the patient and provide the family with a detailed summary of any concerns. If necessary, we can also include basic transportation in this service, ensuring access to essential wound care services and support.

We understand the emotional and physical challenges of living with a terminal illness. Our team is dedicated to supporting both your loved one and your family through a multi-faceted approach. This includes developing personalized care plans that are centered around the patient's wishes, providing emotional support, facilitating life review, and suggesting comfort medications. With a team of seasoned Hospice nurses, we have a deep understanding of the intricacies of the dying process. Our nurses work closely with your Hospice team to provide continuous bedside end-of-life nursing support, which is not covered by insurance-based Hospice care. We specialize in medication management by administering prescribed medications and maintaining open communication with the Hospice team. Furthermore, we create personalized care plans tailored to each patient's needs and offer compassionate wound care services for both the patient and their family during this critical time.

As diseases progress, changes can happen rapidly. We're here to help connect you and your family with the right resources to maximize convenience and comfort through personalized care plans. We collaborate with exceptional companies in the metro area that offer a wide array of services, including caregiving, medication management, wound care services, physical therapy, in-home primary care, in-home phlebotomy, mobile X-rays and imaging, and more.

We offer personalized care plans for the comprehensive in-home care of Foley and Suprapubic catheters. Our services include medication management, such as saline or antibiotic flushes, monthly replacements, and education on management techniques that are specifically designed to prevent urinary tract infections. Furthermore, we provide wound care services to effectively address any related concerns.

We offer daily insulin injections on either a short-term or ongoing basis as part of our comprehensive medication management services. In addition to this, we coordinate with the patient's provider to develop personalized care plans and ensure the most effective insulin regimen. We also recommend long-acting and oral hypoglycemic agents when appropriate, and align our approach with other necessary services, including wound care services.

If you're traveling to Denver for treatment at the Centeno-Schultz Clinic, we offer personalized care plans tailored to your needs. Our services include post-operative transportation and care for the required 4–6 hours after your procedure.
